I grant Opportunity@Work permission to share my submitted photo (optional) and story for the purpose of raising awareness of STARs across the country. Additionally:
1. I understand that my photo and story may be published on Opportunity@Work's websites and on Opportunity@Work's social media channels (including but not limited to Facebook, Instagram, X, and LinkedIn), and Opportunity@Work's email marketing campaigns. Opportunity@Work may use quotes or lines from the story on platforms where the full story will not fit.
2. I understand that the decision to share my photo and story is entirely voluntary, and that there is no obligation to participate and no compensation for participating.
3. I consent to being contacted by email if Opportunity@Work wishes to follow up on this story or future storytelling opportunities.
4. I understand that I have the right to withdraw consent for use of my photo and story at any time, and to have all of my data deleted. I understand that when requesting this it may take several days to remove my photo and story from Opportunity@Work websites and social media channels entirely. To do so, please contact us at digital@opportunityatwork.org.
5. I understand that while Opportunity@Work uses industry-standard security measures to protect my information and takes security very seriously, no system is completely immune to every possible threat. In the event of a data breach, Opportunity@Work will take immediate action to support me without accepting liability for the breach itself by promptly notifying me, investigating the breach, and informing me of steps that the organization will take to restore my trust.
